Click on the links below to access ALSC forms, such as ballot nominee suggestion forms, committee, discussion group, and priority group consultant report forms, and committee volunteer forms.

Some of the forms below are in Microsoft Word format. You can copy and paste each form into a word processing program, or directly save it to your word processing program, then fill it in, and e-mail it back to us at alsccommittees@ala.org. Award and Notable Committee Quarterly Report - (Due by Sept 15, Dec 15, Feb 15, May 15)

  • This form is for the following award and notable committees: Batchelder, Belpre, Caldecott, Geisel, Newbery, Odyssey, Sibert, Wilder, Excellence for Early Learning Digital Media, Notable Children's Books, Notable Children's Digital Media, Notable Children's Recordings. All other committees and task forces should use the form below.
  • Once you submit your report, you will receive an acknowledgement email with a copy of your report. Please be sure to forward a copy of your report to your co-chair (if applicable) and PGC. Additionally, please upload a copy of the email to your group's ALA Connect space and mark it “public.” You can save it as a PDF or copy and paste the email into a word document and upload.  General instructions for uploading files available here: http://www.ala.org/alsc/usingalaconnect.
  • Check your committee's report submission status via the tracker using Airtable.

Committee/Task Force Quarterly Reporting Form  - (Due by Sept 15, Dec 15, Feb 15)

  • All committees and task forces, except those named in the section above, should use this form.

Year-End Process Committee Report and 2017-2018 Work Plan - This form is in lieu of a quarterly report for May 15 (above).

  • All process committees (except Local Arrangements, Nominating, and President's Program) should use this form. Professional awards, and scholarships should use their regular quarterly report above. Local Arrangements, Nominating, and President's Program should use the regular quarterly report for May 15.
  • Please submit your completed form as follows: 1) Email it to alsccommittees@ala.org, with a cc to your Priority Group Consultant; and  2) Upload a copy to your group's ALA Connect space and mark it “public.” You can save it as a PDF or copy and paste the email into a word document and upload.  General instructions for uploading files available here: http://www.ala.org/alsc/usingalaconnect
