This sample evaluation form is for staff to use following a computational thinking local library event.
This evaluation form can be adapted for use in evaluating any type of a library activity. It is useful in helping staff to consider the impact of activities on youth and families. It includes space for library staff to describe the audience served and describe strengths and weaknesses of the event.
