Condition/Damage Report

Libraries are required to complete the condition report form when they receive the exhibit.

3. If there are any items missing from Case #7, the case for kiosk A (i.e., keys, kiosk restore discs, touch up paint, hardware, screws), please notify ALA immediately.

4. If there is no major damage, complete this form within 48 hours and fax or e-mail to the address below. We must have a damage report form on file from each library. Duplicate pages if needed.
