Staff Change Form Instructions

Supervisors fill out this form when any of the following apply:

Arrivals: To be filled out for all new staff who requires network access. Three business days' notice is required for network access setup.

Departures: To be filled out only after you have notified Human Resources and the staff member has confirmed their last day of work.

Leave: To be filled out only after you have notified Human Resources and the staff member has confirmed their dates.

Status changes: Use when an intern or temp becomes FTE or when an FTE becomes a contractor or has any relationship with ALA that still requires access to ALA resources.

Transfers into or out of your unit: both the hiring and former manager should fill out their own version.

HR personnel will fill out this form to notify Financial Systems, IT, MRS, Payroll & Staff Support Services of legal name changes for existing staff so that systems and access can be updated.
