Nomination submission deadline is December 4, 2020.
Electronic submissions are required. All supporting documents should be compiled by the nominator and uploaded as a single PDF document below. Supporting documents should include:
- a nomination letter from you
- a current vita for the person being nominated
- as many letters as you wish from other librarians supporting the nomination
Please also submit a high resolution photo of the nominee (at least 300 dpi), which will be used to make the official announcement.
Applications are kept in the award pool for two years and may be updated each year, as long as they are received by the award deadline.
Please note: the form will not accept submissions with file sizes larger than 20MB. If you are not redirected to a submission confirmation screen and do not receive a confirmation email after you submit your application, then your submission has NOT been received. Please check the size of your file.
