The NEH has requested that all sites distribute and collect audience evaluation forms for Changing America.
The evaluation form is two pages long and can be printed on both sides of one 8 ½ x 11 sheet.
Please distribute this form to exhibition visitors and to participants in your public programs.
Collect the forms and mail or e-mail them at the end of your exhibition display period to:
