Subject Proposal Form
| SUBJECT PROPOSAL FORM (for the Library of Congress Subject Headings)
General instructions: This form can be use to submit proposals for new subject headings to be added to the Library of Congress Subject Headings (LCSH) thesaurus. The information with an asterisk (*) MUST be filled in. The other information in optional. You must list at least ONE publication that is about the subject. There is an example of a filled-in form at the end. Your proposal will be researched by the Subject and Bibliographic Access Committee of ANSS, and you will be contacted concerning the status of your proposal. Most will be submitted to the Library of Congress as a new term or a cross-reference. Some may need modification.
