Please use this form to purchase a ticket or a full table for your state at the 2nd National Advocacy Honor Roll Banquet. The banquet will be held in on Friday, June 24, 2005, at the Hotel Intercontinental Chicago, Renaissance Room, 7:30 – 10:00 pm, during the ALA Annual Conference.
Individual tickets may be purchased for $75. A full table of 8 may be purchased for $540 (a 10% discount). All attendees will receive a copy of the souvenir program book, and advocacy honorees will receive a special recognition pin.
