Sample Book Notes
To accompany that I used quarter sheet pages of 81/2 by 11 copy paper that were each illustrated with commercial clip art and had manuscript writing grid lines and simple categories on them.

The four quarter sheets would then be stapled together. One set of book notes would be along these lines, but with multiple lines and clip art
For other lessons I sometimes used third of a page Book notes or half page Book Notes. The first grade students enjoyed the little note taking booklets tremendously.
