- Include an evaluation form in the packet. Have participants complete the forms before leaving. Request the cooperation of the audience both at the beginning and the end of the event. List a committee member's name and address at the bottom so participants can return by mail if necessary.
- Suggestions for an evaluation instrument include keeping it to one page, printing on bright paper, and including some open-ended questions. See

- ACRL may also ask to distribute a separate evaluation form. Check with ACRL to see if they want to include one in the packet.
